研究问题

Most studies report an association between living alone and increased mortality []; however, one study suggests that hospitalization rates may decrease among older individuals living alone [].;One study suggests that among individuals living alone, those who feel lonely have higher mortality []; however, a contradictory study revealed that when adjusting for factors such as older age or health problems, loneliness does not lead to increased mortality [].;In this study, we aimed to compare and analyze all-cause, cancer, and cardiovascular mortality between older adults living alone and those living with others.

方法

The sampling frame of the KLoSA comprises enumeration districts clustered in 15 metropolitan cities and provinces and stratified first by type of area (urban/rural) and then by type of housing, as identified by the National Statistical Office’s 2005 Census.;We included an initial population of 5,549 participants aged 60 years or older in 2006.;We excluded 850 participants whose household composition changed during the follow-up period (e.g., from living alone to living with others, or vice versa), as such transitions could confound the association between living arrangement and mortality.;Therefore, these participants were excluded from the analysis.

核心结论

After IPTW adjustment, no significant SMDs were observed between adults living alone and those not living alone.;1 presents the results of the sensitivity analysis examining factors associated with the HRs for all-cause, cancer, and CCV mortality.;Adults over 70 years and low body weight showed significantly increased mortality risk across all-cause, cancer, and CCV disease. ‖ 结论:In this study, we highlight that community-dwelling older adults living alone did not experience increased all-cause, cancer, or cardiovascular mortality over a 16-year follow-up period, even after adjusting for socioeconomic status (SES) factors.

局限

To minimize confounding effects, only individuals aged 60 years or older were included, while those diagnosed with cardiovascular disease, stroke, or cancer were excluded.;This cohort effect may be an unexpected confounder because of the differing psychological backgrounds [].;This study has several limitations that should be acknowledged.
