研究问题
However, the direction and magnitude of their associations with psoriasis risk remain uncertain.;A central challenge is distinguishing associations related to medication exposure from those related to the underlying conditions that lead to treatment.;Diabetes, obesity, insulin resistance, dyslipidemia, cardiovascular disease, and chronic kidney disease are associated with psoriasis and may also influence whether glucose-lowering or lipid-lowering therapy is prescribed and which medication is selected.
方法
We also examined whether the findings varied according to medication class, comparator, outcome definition, and study design.;We included longitudinal observational studies according to predefined eligibility criteria to evaluate the association between glucose-lowering or lipid-lowering medication use and either incident psoriasis or first-recorded psoriasis.;For this review, incident psoriasis referred to a newly diagnosed outcome during follow-up in studies that explicitly excluded previously recorded psoriasis before baseline or during a specified pre-index period.;Eligible study designs included cohort, case-control, and nested case-control studies.
核心结论
Most studies used propensity score matching, propensity score weighting, or multivariable regression to control for confounding.;Comparators included other active medications, heterogeneous treatment populations, medication non-users, and groups defined by different exposure durations, intensities, or levels of adherence.;The studies differed substantially in comparator selection, time-zero specification, exposure definition, follow-up rules, and psoriasis outcome definition, and some reports may have drawn on the same or partially overlapping database populations. ‖ 结论:These findings provide background plausibility but do not establish prevention of cutaneous psoriasis or a shared preventive effect across all TZDs.
局限
This systematic review included 18 longitudinal observational reports.;These limitations, together with uncertainty about evidence independence, preclude a stable interpretation of the findings as medication-class effects.;Several Taiwan reports may also have included overlapping populations, limiting their interpretation as independent replications.
