研究问题

By integrating these perspectives, this review aims to provide a structured understanding of the clinical complexity of multimorbidity in older adults with diabetes and to support the transition from disease-centered care toward individualized, integrated, and function-oriented management.

方法

Methodology This narrative review was conducted to synthesize current evidence on the clinical characteristics, health consequences, management challenges, and comprehensive management of multimorbidity in older adults with diabetes.

核心结论

21 , 22 Collectively, these findings suggest that multimorbid conditions do not exist as isolated entities but instead constitute an interconnected pathological continuum.;Taken together, these findings indicate that pharmacotherapy-centered management approaches are no longer sufficient to address the complex healthcare needs of older adults with diabetes.;These findings suggest that standardized patient education and routine follow-up alone are insufficient to meet the complex needs of this population.

局限

Furthermore, pain and mobility limitations caused by osteoarticular disorders reduce physical activity, accelerate the progression of frailty, and increase the risks of falls, fractures, and disability.;However, current clinical guidelines are largely developed for individual diseases and provide limited guidance for integrated management, resulting in fragmented clinical decision-making and challenges in implementation.
