研究问题
Acute findings, however, should not be automatically extrapolated to chronic post-COVID symptoms because the relevant populations, time points, treatments, and competing causes differ.
方法
A DGBI is a positive clinical diagnosis based on a characteristic symptom pattern and accepted criteria after reasonable consideration of alternative organic disease; a symptom such as post-COVID diarrhea is initially a descriptive phenotype, not a diagnosis [,].;Diarrhea, nausea/vomiting, abdominal pain, and anorexia were documented in early hospitalized and ambulatory cohorts, and viral RNA was detectable in stool in a substantial subset of patients [-].;We hand-checked reference lists of key systematic reviews, major cohort studies, randomized trials, and clinical guidelines for additional relevant publications.
核心结论
Patients may report persistent or new diarrhea, abdominal pain, nausea, altered appetite, dyspepsia, reflux, constipation, or other bowel symptoms after the acute infection [-].;These symptom reports are clinically important, but they are not synonymous with a functional bowel disorder or disorders of gut-brain interaction (DGBI).;This review focuses on persistent or new-onset diarrhea after COVID-19 while placing it within the wider post-COVID GI spectrum. ‖ 结论:Dedicated, well-controlled post-COVID diarrhea trials are needed before specific microbiome, antiviral, immune, or barrier-directed therapy can be recommended.
局限
Eligible evidence included adult human observational studies, randomized controlled trials, systematic reviews/meta-analyses, and major clinical guidance relevant to epidemiology, mechanisms, diagnosis, or treatment.;Effect estimates from the strongest studies are reported where available, and the limitations of this approach are addressed below.
对本项目的价值
★☆☆☆☆ 菌群是可远程部署、成本最低的干预变量,但因果性与个体化响应仍是主要瓶颈。设计 meta-analysis;最大样本量约 1,000。
