研究问题

Although previous studies have separately examined the relationships of lactate with metabolic abnormalities and of biological aging with liver diseases, to our knowledge, no prospective population-based study has systematically evaluated whether accelerated biological aging mediates the associations of plasma lactate and the glucose-lactate index with incident MASLD.

方法

This study utilized data from the UK Biobank, a large-scale, population-based prospective cohort that recruited approximately 500,000 individuals aged 37–73 years between 2006 and 2010 [23].;At baseline, participants provided detailed demographic, lifestyle, and health-related information, underwent physical assessments, and donated biological samples.;Ethical approval was granted by the North West Centre for Research Ethics Committee (Reference: 11/NW/0382), and all participants provided written informed consent.;Among 502,359 participants, 123,830 with missing data for biological aging indicators (KDM-BA Accel, PhenoAge Accel, or rLTL), 202,515 without (¹H-NMR)-based metabolomic data, and 402 with baseline MASLD were excluded.

核心结论

Overall, the mean age was 56.48 years, 46.0% of participants were male, 95.9% were White, and 90.8% resided in England.;Among the 2,297 participants who developed incident MASLD, 48.9% were male, and 14.6% were current smokers.;Among the 173,315 participants who remained MASLD-free, 46.0% were male, and 9.8% were current smokers. ‖ 结论:This study showed that higher plasma lactate and the glucose-lactate index were associated with an increased risk of incident MASLD, with these associations partially mediated by accelerated biological aging.

局限

Recent evidence also linked LDH to biological age and suggested that it may partially mediate the association between inflammation and biological aging, providing preliminary support for the involvement of lactate-related metabolic processes in aging [39].;Whether lactate or the glucose-lactate index can further improve risk prediction beyond conventional MASLD risk factors warrants evaluation in future studies using measures such as the C-statistic, net reclassification improvement, integrated discrimination improvement, and external validation.;Future studies should examine whether modulating lactate levels could influence DNA methylation patterns associated with epigenetic aging.
