研究问题
In this study, our aim was to examine national trends in HHC utilization, economic-related inequality, and horizontal inequity among older adults in China from 2016 to 2023, with attention to urban-rural and regional differences in health system capacity and socioeconomic context.；To evaluate temporal patterns in service utilization, economic-related inequality, and horizontal inequity, we primarily described changes across the four survey waves and calculated absolute changes between 2016 and 2023.；Policy initiatives such as cross-regional medical insurance settlement systems and long-term care insurance pilot programs aim to improve healthcare access and financial protection, particularly for older adults with functional limitations.
方法
Existing Chinese studies have examined service availability for specific components, such as psychological counseling and home visiting services, during 2005–2018, 9 , 10 HHC utilization during 2016–2018, 21 or cross-sectional patterns in 2018.；Methods Study design This serial cross-sectional study used the 2016, 2018, 2020, and 2023 waves of the China Longitudinal Aging Social Survey (CLASS), a nationwide survey that uses stratified, multistage probability sampling and questionnaire interviews.；Each survey-wave record was treated as a repeated cross-sectional observation.

Together, these results indicate increasing pro-rich economic-related inequality and horizontal inequity.；MICE analyses using the full annual analytic samples showed the same direction of increasing pro-rich inequality (concentration index: 0.03 in 2016 to 0.27 in 2023; 2023–2016 difference, 0.24 [95% CI: 0.18–0.30]) and inequity (horizontal inequity index: 0.04–0.22; difference, 0.18 [95% CI: 0.13–0.24]), supporting the robustness of the main conclusions.；18 , 19 , 20 , 45 In China, we observed a similar pro-rich pattern.

9 The availability of community-based home visiting services rose from 9.7% in 2005 to 33.7% in 2018; however, only about one third of older adults reported local availability, with persistent geographic disparities.；Policy initiatives such as cross-regional medical insurance settlement systems and long-term care insurance pilot programs aim to improve healthcare access and financial protection, particularly for older adults with functional limitations.
