研究问题
The three living states were defined according to whether participants reported frequent pain located in the back and, if so, its severity.;Based on previous evidence, we hypothesized that greater chronic disease burden would be associated with higher hazards of low back pain onset and worsening, lower hazards of recovery, and higher hazards of death.;Definition of low back pain states Low back pain states were operationalized at each wave using HRS questionnaire items on whether respondents were frequently troubled by pain, the location of the pain, and the reported overall pain severity.
方法
Thus, cross-sectional analyses and binary outcome models may fail to capture the dynamic processes of onset, progression, recovery, and death [ 10 , 11 ].;The HRS is a nationally representative longitudinal cohort of middle-aged and older adults in the United States.;Interview records collected when participants were aged 60 years or younger were excluded, whereas participants could enter the analytic cohort at their first subsequent interview after age 60.
核心结论
Together, these findings suggest that chronic disease burden may contribute to a less favorable course of low back pain.;These findings suggest that low back pain in older adults is not merely a localized musculoskeletal condition [ 25 ], but often coexists with accumulated chronic disease, functional decline, and poor mental health [ 26 ].
局限
In older adults, low back pain is associated with impaired quality of life, activity limitation, functional decline, depressive symptoms, greater health care use, and increased mortality risk [ 3 – 5 ].;Multimorbidity is prevalent among older adults [ 12 , 13 ] and may influence the course of low back pain through pathways involving chronic inflammation, metabolic dysregulation, reduced physical activity, muscle weakness, functional limitation, and psychological burden [ 14 , 15 ].
