研究问题

The highly aggressive nature, strong invasiveness, high mortality rate, and challenges in early detection collectively contribute to its poor clinical outcomes, posing a significant challenge to global public health systems.;Despite advances in understanding CCA biology, CCA remains frequently under-recognized and is diagnosed late in routine clinical practice.;Together, these gaps underscore an unmet need for non-invasive approaches that could identify at-risk individuals earlier and complement, rather than replace, existing diagnostic modalities.

方法

This multicenter, prospective cohort study was designed to systematically characterize the multi-kingdom gut microbiota in patients with CCA.;To ensure the robustness and generalizability of our findings, we established four independent cohorts across three distinct geographical regions of China (Eastern, Central, and Northwestern China).;All study procedures adhered to the principles of the Declaration of Helsinki and were approved by the Institutional Review Boards of each participating center.;The primary cohort was recruited from the First Affiliated Hospital, School of Medicine, Zhejiang University (Eastern China).

核心结论

Sex and age did not differ significantly between the CCA and HC groups within any cohort, minimizing demographic confounding.;To ensure the two-batch, multi-period collection did not confound our findings, we quantified batch (collection-period) effects before and after correction (Additional file : Figs. , ).;Before correction, batch showed a significant but small effect across all seven layers (R^2 = 0.6%–3.0%), consistently smaller than the disease-group effect. ‖ 结论:In conclusion, through large-scale metagenomic sequencing, this study provides the first comprehensive landscape of the gut multi-kingdom microbiota (encompassing archaea, bacteria, fungi, and viruses) and functional attributes (including KO genes, KEGG pathways, and CAZymes) in CCA.

局限

Their presence in controls may reflect complex host-microbe interactions that warrant further investigation.;Future studies should seek to validate these markers in prospective cohorts and explore their mechanistic roles in CCA development.;This complementary role is clinically relevant because current approaches each have important limitations [–]: CA19-9 lacks sensitivity, is falsely elevated in benign biliary obstruction, and is undetectable in Lewis antigen-negative individuals; cross-sectional imaging can misclassify iCCA as HCC and struggles to distinguish malignant from benign strictures; and tissue-based methods such as brush cytology, although highly specific, have limited sensitivi

★☆☆☆☆ 菌群是可远程部署、成本最低的干预变量,但因果性与个体化响应仍是主要瓶颈。
