研究问题
This gap in knowledge impedes the development of targeted interventions and therapeutic strategies to effectively treat and prevent this devastating condition.;Previously, kwashiorkor had been attributed to protein deficiency []; however, emerging evidence suggests that its etiology is multifactorial, involving a complex interplay of nutritional, and environmental factors.;However, despite finding aflatoxins in biological samples of children with kwashiorkor [–], a causal link has not be demonstrated as the possibility of reverse causality has not been ruled out [].
方法
Participants were matched by age categories, sex, and triage score as a proxy of clinical severity, with children classified into groups based on edema levels (mild, moderate, and severe), which served as an indicator of kwashiorkor severity (see Table ).;Biological samples were collected at various time points []: upon recruitment for all study participants [], at discharge from inpatient care, and [] at exit from outpatient care (known in this context as an ambulatory therapeutic feeding center, ATFC).;This study recruited children with severe acute malnutrition aged 6 to 59 months eligible for new admission to either inpatient care in the ITFC of the Madarounfa District Hospital or enrolment in outpatient care in one of the ATFC supported by MSF in the region of Madarounfa.
核心结论
NAM children were more likely to test positive for malaria using rapid diagnostic tests compared to children with kwashiorkor or marasmus and they also presented a higher Pediatric Early Warning Scores (PEWS) at baseline.;FA reduced the proteomics data into 9 proteome-LVs (pLV).;Out of 9 pLVs, 7 were found to be different in kwashiorkor (pLV 1, 3, 4, 5 and 7 were higher in kwashiorkor, pLV2 and 9 were lower in kwashiorkor than non-kwashiorkor cases), and hence passed criterion 1. ‖ 结论:These observations suggest that severe malnutrition, and particularly kwashiorkor, is not simply a state of nutrient deficiency but a multifaceted physiological condition that reconfigures systemic molecular interactions.
局限
Previous protein-kinetic studies have demonstrated markedly reduced whole-body protein synthesis in children with kwashiorkor [], making a generalized increase in protein synthesis a less likely explanation, although selective synthesis of individual proteins cannot be excluded.;However, gut microbiome differences between children with kwashiorkor or marasmus and the NAM group cannot be excluded, given the small sample size, similar living conditions, and limited gastroenteritis cases.;Future larger studies are therefore warranted.
