研究问题
Falls are one of the major health problems in older adults [1].
方法
Studies covering a wider age range were eligible only if an analysis restricted to participants aged 60 years and older was presented.;Criterion I3 admitted asymmetry measured either before the occurrence of the outcome or at the same time.;The search was not repeated before submission of the manuscript; consequently, studies published after 2 August 2026 could not be included.;The included studies were checked for multiple reports from the same sample.
核心结论
A single prospective cohort, the Osteoporotic Fractures in Men Study [22], contributed 53% of all participants, and the two largest studies [21,22] together contributed 66%.;Three studies applied threshold values set a priori: 15% in one [37] and 20% in two [31,33].;In the same cohort, by contrast, handgrip strength asymmetry was associated with recurrent falls (relative risk 1.20; 95% CI 1.01–1.43) and with hip fracture (hazard ratio 1.41; 95% CI 1.02–1.96), while handgrip strength asymmetry and lower-limb power asymmetry remained statistically independent (r = 0.01). ‖ 结论:These findings remain limited by the observational nature of the studies and the very low certainty of evidence in all comparisons assessed.
局限
[42], studying 45 older adults with mobility limitation, found that lower-limb power explained 2–8 percentage points more of the variance in functional outcomes than strength in adjusted models.;Persistence of an association after adjustment does not prove causality, and its attenuation does not in itself identify a mechanism.;In such studies, changes in the gait pattern may have preceded a fall, but they may also have been a consequence of injury or activity limitation.
