研究问题
Whether exercise-related changes in dynamic functional connectivity (dFC) reflect neural reorganization, vascular effects, or both remains uncertain, and most studies have not tested sex as a moderator.
方法
We propose the Sex-Dependent Dynamic Reconstitution Model (SDDRM) as a hypothesis-generating framework.;We outline four candidate, sex-stratified predictions: (1) sex may moderate exercise-related MTL flexibility changes, with menopausal status as an exploratory moderator; (2) dynamic flexibility changes may partially mediate cognitive improvement, with indirect pathway magnitudes compared using moderated mediation; (3) exercise cessation may produce different flexibility decay trajectories by sex; and (4) exercise modality may interact with sex.;We emphasize dFC methodological caveats—including scan duration, motion, test–retest reliability, and neurovascular coupling—and distinguish biomarker-confirmed AD from MCI and cognitively unimpaired populations.
核心结论
Recent large-scale studies have mapped stage-dependent dynamic network dysfunction across the AD continuum [12], and animal models have demonstrated that physical activity can modulate network-level dynamics in AD-vulnerable circuits [13].;In AD, emerging evidence suggests reduced flexibility, altered variability, and increased state persistence [9–12], but these findings are methodologically heterogeneous and should not be interpreted as evidence that dynamic measures are inherently superior to static measures.;We conclude with testable predictions, challenge conditions, and a research framework.
局限
Keywords: Alzheimer's disease, dynamic functional connectivity, sex differences, exercise intervention, network flexibility, hypothesis-generating framework, methodological limitations 1.;What this static framework obscures, however, is a fundamental property of neural organization: functional connectivity is not stationary but fluctuates dynamically over timescales of seconds to minutes [7].;The neural mechanisms linking exercise to cognitive preservation, however, remain incompletely characterized.
