研究问题

16 , 17 Little is known about how these conversations unfold in real‐world practice.;Existing evidence characterizes treatment outcomes, risks, and clinician‐reported communication practices, but little is known about how AAT discussions unfold in real‐world clinical encounters using direct observation.;1.1.3 Qualitative analysis We used qualitative content analysis 19 of encounter transcripts to examine clinician communication about AAT.

方法

We conducted an exploratory qualitative study of audio‐recorded AAT counseling encounters.;1.1 Methods 1.1.1 Design We conducted a qualitative study of real‐world AAT counseling encounters in neurology and memory clinics at two academic medical centers in the northeastern United States.;Clinical encounters of enrolled participants were audio recorded using in‐room devices set up by the CRC, who was not present during visits.

核心结论

These findings demonstrate that SDM for AAT may be influenced by communication framing.;During the coding process, we identified candidate categories that were refined to reflect broader themes across encounters.;Our findings also suggest variation in how clinicians explored patient values and priorities and translated potential treatment effects into personally meaningful outcomes.

局限

However, communication patterns—including optimistic framing, variable elicitation of patient values, inconsistent teach‐back, and limited discussion of non‐treatment alternatives—may shape decision making.;Several clinicians helped bridge this gap by discussing what treatment might mean for independence, function, and valued activities, while emphasizing that individual treatment outcomes cannot be predicted.
