研究问题
For older adults who are faced with (sub)acute medical condi- tions and functional declines, admission to a geriatric reha- bilitation (GR) facility may be required [1].
方法
In this paper, we present the results of the comparative outcome evaluation, to assess the effect of the Better@ Home program on the level of independence in activities of daily living (ADL) from GR admission to discharge in the prospective Better@Home cohort, compared to a histori- cal control group receiving conventional inpatient GR.;All pro- cedures in this study involving human participants are in accordance with the recent version of the Declaration of Hel- sinki [22].;Written informed consent of all participants was obtained during inpatient rehabilitation before study inclu- sion and baseline assessment in the Better@Home cohort.;The Better@Home study incorporated a historical control population, consisting of patients who previously received conventional inpatient GR.
核心结论
Mean inpatient GR duration was 33.1 days in the Better@Home group and 37.6 days in the control group (mean difference 4.5 days; 95% CI −1.7 to 10.7; p = 0.151), whereas mean total rehabilitation duration was 64.8 and 37.6 days, respectively (mean difference 27.3 days; 95% CI 20.4 to 34.1; p < 0.001).;Almost all patients (97%) received treatment sessions at home, 76% of patients received remote sessions and 14% received outpatient sessions.;At GR discharge, 35% of participants had a BI score of 19 (n = 19 Better@Home, n = 25 control) or 20 (n = 21 Better@Home, n = 53 control), indicating a potential ceiling effect. ‖ 结论:These findings therefore suggest that the European Geriatric Medicine Better@Home program may be a valid alternative to con- ventional inpatient GR, achieving comparable outcomes of independence in activities of daily living.
局限
Additionally, selection bias may have been introduced by excluding historical control patients who received home- based GR.;Conclusions and future research The Better@Home study demonstrates that a structured home-based GR program, replacing part of inpatient GR, can achieve comparable levels of independence in activities of daily living to conventional inpatient GR.;Future studies in the field of home-based GR should include broader outcome domains such as IADL and par- ticipation measures, as it remains uncertain whether the Bet- ter@Home program may have achieved superior outcomes in these domains [34].
