方法

METHODS: We synthesized recent systematic reviews, meta-analyses, randomized screening trials, and population-based registry data, supplemented by landmark historical studies.;We searched PubMed/ MEDLINE and, where relevant, Embase for English-language articles published up to June 2026, using combinations of the terms “prostate cancer,” “epidemiology,” “incidence,” “mortality,” “prevalence,” “risk factors,” “screening,” “PSA,” “MRI,” and “dispa- rities.” We prioritized (i) contemporary international guidelines, (ii) large randomized controlled trials and their long-term follow-up, (iii) population-based registry analyses and pooled/multi-cohort studies, and (iv) recent systematic reviews and meta-analyses, supplemented by landmark historical studies where they remain foundational.

核心结论

CONCLUSIONS: Progress will depend on preventing metastatic disease, minimizing overtreatment, and implementing modern diagnostics equitably.;These trends show that prostate cancer epidemiology cannot be separated from early-detection policy, and they support risk-adapted approaches, including biomarker- informed pathways and MRI, applied equitably across higher-risk populations.;Consequently, age-related mortality rates have decreased substantially over the last three decades in North America [6], Europe [7] and other high income countries of Australasia [8]. ‖ 结论:CONCLUSION By 2026, prostate cancer epidemiology reflects how health systems detect and manage disease as much as biology itself.

局限

Incidence, mortality and prevalence GLOBOCAN 2022 shows marked geographic heterogeneity in incidence and mortality across Europe (473,011 cases; 115,182 deaths), Asia (386,424; 120,485), North America (255,782; 39,605), Latin America/Caribbean (225,985; 61,056), Africa (103,050; 55,744), and Oceania (23,602; 5358) [1].;Most modifiable exposures, by contrast, rest on observational data of uncertain causality and are more appropriately situated within general cardiometabolic health than framed as prostate cancer–specific preventive targets, while a residual group remains hypothesis-generating.;Where randomized and observational estimates are considered jointly, we specify the design and evidenti
