研究问题
Each full‐text was categorized according to whether the sample consisted of one or more of CN participants or participants with subjective cognitive decline (SCD), MCI, or dementia.;Relevance reflected whether NPS were a central focus of the study; novelty reflected the contribution of new findings, measures, biomarkers, or approaches; and potential impact reflected the importance of the findings for advancing the understanding, assessment, or management of NPS.;In a study of 338 cognitively unimpaired older adults, three distinct participant clusters were identified based on Stage 2 features of mild objective cognitive dysfunction, SCD, and the presence of NPS. 38 The cluster characterized by objective cognitive dysfunction combined with SCD showed the highest rate of amyloid positivity (47.6%), suggesting that this specific combination of early symptoms is most strongly associated with the AD continuum.
方法
These instruments may measure global neuropsychiatric symptomatology (e.g., Neuropsychiatric Inventory‐Questionnaire [NPI‐Q] 18 ) and may support a clinical diagnosis of a neuropsychiatric syndrome but are insufficient on their own.;Here, we synthesize relevant literature on NPS across the AD continuum and integrate these recent findings within the current AD clinical staging model.;Together, these perspectives aim to refine our understanding of the complex interplay between NPS, neurodegenerative pathology, and clinical outcomes, ultimately informing future research and clinical practice.2METHODSIn accordance with methodological standards for narrative reviews, we adhered to the reporting standards indicated by the Scale for the Assessment of Narrative Review Articles (SANRA). 19 .
核心结论
The bidirectional arrow between behavioral and cognitive impairment indicates that behavioral changes may precede cognitive decline, follow it, or emerge concurrently.;For example, although widely used scales such as the Geriatric Depression Scale (GDS) demonstrate overall reliability, a recent study found notable ethno‐racial differences in symptom endorsement. 25 Similarly, the MBI‐C has been translated and validated in multiple languages including Korean, Chinese, Spanish, and German, and more recently, found to be a valid and reliable tool for the Turkish population. 26 Lastly, establishing appropriate cutoffs for validated NPS scales remains important to improve the assessment of symptom severity.;The next few sections will describe current research (2024 and 2025 studies only) demonstrating changes in NPS prevalence and presentation, discrepancies in NPS reporting between self‐ and SP‐based assessments, and relationships between NPS and cognitive and pathological outcomes across the AD continuum in the context of the current AD clinical staging model.
局限
Future studies should align NPS assessments with standardized diagnostic criteria to improve consistency across research studies and facilitate translation into clinical practice.3.2NPS across the AD continuum and clinical staging modelAs our understanding of pathological and cognitive changes in neurodegenerative diseases has evolved through revised staging criteria, there is a growing need to examine how NPS manifest across disease stages (Figure 2).;AD, Alzheimer's disease; BPSD, behavioral and psychological symptoms of dementia; MBI, mild behavioral impairment; MCI, mild cognitive impairment. It is important to note that the AD clinical staging criteria formally apply to individuals with biomarker‐confirmed AD; however, many studies reporting on NPS in CN, SCD, or MCI participants have not required biomarker confirmation.;Collectively, these findings suggest that specific NPS (e.g., depression, irritability, agitation, and anxiety) may emerge before measurable cognitive decline and may represent early manifestations of underlying AD pathology, rather than a consequence of cognitive decline; however, findings differed regarding the relative contributions of Aβ and tau pathology.
