研究问题

Cardiovascular disease (CVD) remains to be the leading cause of death worldwide, accounting for an estimated 19.8 million deaths each year, which accounts for approximately 32% of all global deaths [].;Nevertheless, the joint association between LDL-C and inflammation on all-cause and cause-specific mortality remains inadequately understood without conclusive findings.;To address this research gap, the present study aimed to evaluate the associations of LDL-C, inflammation, and their joint associations with all-cause and CVD mortality by using data from the National Health and Nutrition Examination Survey (NHANES) and the Chinese Longitudinal Healthy Longevity Survey (CLHLS).

方法

Participants who were younger than 20 years old, or pregnant, or with missing data/outliers on LDL-C/neutrophil count/platelet count/lymphocyte count/mortality were excluded.;At last, a total of 18,416 participants were analyzed.;Participants were recruited from 22 provinces in China in the CLHLS.;Participants who were with missing data/outliers on LDL-C/platelet count/lymphocyte count/mortality were excluded.

核心结论

The differences for age, race, education level, marital status, current drinking, LDL-C, SII, NCDs, lipid-lowering drugs, all-cause mortality, and CVD mortality in the NHANES, and age, education level, marital status, BMI categories, current drinking, LDL-C, PLR, NCDs, and all-cause mortality in the CLHLS were statistically significant across the four groups classified by LDL-C and SII/PLR (all P-values < 0.05), respectively.;The combined effects of LDL-C and SII on all-cause/CVD mortality were statistically significant, with S (95%CIs) of 0.23 (0.22–0.25) and 0.31 (0.29–0.32) for mortality from all-cause and CVD, respectively.;Although the relationship between SII and CVD mortality was not statistically significant, the HRs (95%CIs) for participants with SII in T3 were 1.46 (1.07–1.98) for CVD mortality compared with participants with SII in T2. ‖ 结论:Our findings provide practical implications for public health interventions and clinical practice, emphasizing the importance of integrating lifestyle modifications with targeted management of LDL-C and inflammation to improve health outcomes.

局限

A key limitation of these studies is that the inflammation was assessed by high-sensitivity C-reactive protein (hs-CRP), a marker that requires specific clinical requests for measurement and is not universally available in routine clinical practice.;The strengths of the present study included: First, to the best of our knowledge, this is the first study to systematically investigate the influence of PA on the associations of LDL-C and inflammation with all-cause and CVD mortality, filling a critical gap in the current understanding of these interrelated factors; Second, validation across two independent cohorts (the NHANES and the CLHLS) ensured the reliability and generalization of our find

对本项目的价值

★☆☆☆☆ 炎症衰老是几乎所有慢性老年病的公共通路,也是最有希望做「抗炎但保留免疫」解耦的靶点。队列:CLHLS、NHANES;最大样本量约 150,047。
