研究问题
Geroscience investigates whether targeting fundamental mechanisms of aging can delay age-related diseases and preserve physiological resilience.;Whether these aging-related mechanisms substantially contribute to the clinical benefits of established HF therapies in humans remains uncertain.;An important question is whether skeletal muscle dysfunction merely reflects advanced HF or actively contributes to its progression.
方法
In the OPTIMISE HFpEF cohort, frailty was associated with a more than fourfold higher risk of the one-year composite endpoint of all-cause hospitalization or death [ 7 ].;In 8,558 participants from the PREVEND cohort followed for 25 years, hypertension, hypercholesterolemia, obesity, smoking, atrial fibrillation, chronic kidney disease, myocardial infarction, and type 2 diabetes collectively accounted for approximately 46–71% of incident HF cases, depending on sex and HF phenotype.;Clinical (interventional): The Q-SYMBIO trial suggested a potential benefit of CoQ10 supplementation in chronic HF; however, the study was relatively small and was conducted before the widespread implementation of contemporary foundational medical therapy [ 26 ].
核心结论
In 8,558 participants from the PREVEND cohort followed for 25 years, hypertension, hypercholesterolemia, obesity, smoking, atrial fibrillation, chronic kidney disease, myocardial infarction, and type 2 diabetes collectively accounted for approximately 46–71% of incident HF cases, depending on sex and HF phenotype.;These findings indicate that frail patients may derive comparable relative, and potentially greater absolute, benefit from effective therapy.
局限
Given its high prevalence, prognostic relevance, and potential modifiability, routine frailty screening should be considered in all patients with HF aged ≥ 70 years, irrespective of ejection fraction or care setting.;Clinical (interventional): The Q-SYMBIO trial suggested a potential benefit of CoQ10 supplementation in chronic HF; however, the study was relatively small and was conducted before the widespread implementation of contemporary foundational medical therapy [ 26 ].
