研究问题
The purpose of this study was to identify clinical and healthcare utilization characteristics in the 1-year prior to a new diagnosis of HF, characterize symptoms in the 4 weeks prior to a new diagnosis of HF, and quantify sex differences therein.MethodsStudy Design and SampleThis was an observational study designed to quantify sex differences in symptoms and clinical characteristics prior to diagnoses of HF (Fig. 1, Visual Take Home Graphic).;The FRAIL scale is a simple and appropriate measure of frailty when objective measurement components (eg, grip strength, walking speed) are not feasible for all study participants, and it has been externally validated in chronic HF.14 Participants responded to the 5 FRAIL scale questions at the time of enrollment (Fig. 3).;Participants were asked whether they had visited any health care provider in the year prior to diagnosis.
方法
Eligible patients were screened, approached, and recruited between January 2023 and November 2024 (Fig. 2).;Adult patients with new diagnoses of HF were identified through an electronic health record (EHR) cohort-identification tool called the Research Data Warehouse (RDW), hosted by the Oregon Clinical and Translational Research Institute’s Clinical Research Informatics program, and through researchers’ chart reviews of outpatient cardiology clinics and inpatient admission records.;The RDW contains EHR data that can be queried to identify potential research participants.
核心结论
A higher PROMIS T-score indicates that more of the symptoms are being measured.;We found that nearly half of the participants were frail, and only 13 (44.4% female) were robust.;We found significant sex differences in frailty rates, with over half of females being frail compared to less than one-third of males (χ2(1) =8.36; P = 0.004).Symptom CharacteristicsSelf-reported symptoms in the 4 weeks leading up to HF diagnosis showed a notable burden of co-occurring symptoms in both females and males, with few sex differences (Table 1).
局限
Whether this reflects higher symptom burdens, increased health-care-seeking behaviors, or the clinical consequences of frailty warrants further study.;Limitations include the cross-sectional design, recruitment from a single academic medical center with a predominantly non-Hispanic white population, and potential recall bias.;However, it is possible that factors related to clinical and HF characteristics, such as the number and type of comorbidities and the burden of overlapping symptoms, impacted the accuracy of symptom and frailty measures. In addition, this study is 1 of the first to measure frailty in newly diagnosed HF.
