When combined with simple functional measures such as grip strength, BIA-based assessment could provide a feasible screening approach to identify vulnerable patients3, 4).We therefore conducted a retrospective pilot study to evaluate whether sarcopenia-related categories assessed by BIA and grip strength are associated with overall survival in mUC patients treated with chemotherapy.;We hypothesized that categories reflecting impaired muscle function (dynapenia and sarcopenia) would demonstrate poorer survival compared with patients with preserved function.Materials and MethodsStudy design and settingThis retrospective observational study included consecutive patients with metastatic urothelial carcinoma treated with systemic chemotherapy at Juntendo University Hospital between January 2014 and February 2021.;In addition, detailed information on chemotherapy regimens, dose intensity, and treatment-related toxicity was not incorporated into the present analysis and should be evaluated in future studies.Future research should validate these findings in prospective multicenter cohorts, assess whether BIA plus grip strength improves prognostic stratification beyond established clinical factors, and determine whether longitudinal changes during treatment add predictive value3, 4).

核心结论

We hypothesized that categories reflecting impaired muscle function (dynapenia and sarcopenia) would demonstrate poorer survival compared with patients with preserved function.Materials and MethodsStudy design and settingThis retrospective observational study included consecutive patients with metastatic urothelial carcinoma treated with systemic chemotherapy at Juntendo University Hospital between January 2014 and February 2021.;Overall, patients in the muscle function impairment group tended to be older than those in the reference group.;Collectively, these findings suggest that impaired muscle function co-occurred with features consistent with frailty-related vulnerability in this cohort.

局限

However, routine clinical assessments may not capture the multidimensional vulnerability often described as frailty.Sarcopenia refers to a loss of skeletal muscle mass and function and has been linked to adverse outcomes in multiple cancers1, 2).;Exploratory analyses separately estimate hazard ratios for dynapenia and sarcopenia versus the reference group; these subgroup estimates should be interpreted cautiously due to small sample sizes.;Given the small sample size and the exploratory nature of the analyses, these findings should be interpreted as hypothesis-generating; however, the direction and magnitude of effect were consistent across models.Prior work in urothelial carcinoma has mainly evaluated CT-derived measures of skeletal muscle quantity, such as psoas area or L3 muscle index, and has linked low muscle mass to survival and treatment-related toxicity9, 10).
