研究问题
The brain age gap (BAG), defined as the difference between predicted brain age and chronological age, provides an integrative index of distributed structural deviations relative to a normative aging trajectory [].;Whether these structural changes reflect an alteration in the brain’s aging trajectory, rather than static disease-related differences, has become a question of growing interest.;Which clinical factors truly drive individual variation in brain aging within the migraine population therefore remains an open question.
方法
Patients with migraine were enrolled consecutively according to the International Classification of Headache Disorders, 3rd edition (ICHD-3) criteria [], with diagnoses confirmed by a board-certified neurologist through standardized clinical interviews and neurological examinations.;Volunteers without a history of migraine, neurological or psychiatric disorders, or major systemic diseases were recruited from community advertisements and screened through structured clinical interviews conducted by neurologists.;Exclusion criteria for both groups included age below 18 years, pregnancy or lactation, secondary headache disorders, serious systemic illnesses, cognitive impairment, major psychiatric conditions requiring hospitalization, concurrent primary headache disorders, and contraindications to MRI scanning.
核心结论
The two groups were well-matched for age (p = 0.105), sex (p = 0.231), years of education (p = 0.210), and body mass index (p = 0.350).;In contrast, patients with migraine exhibited significantly higher levels of psychological distress and sleep disturbance, with elevated scores on the BDI-II (p = 0.014), HADS-A (p = 0.001), and PSS (p < 0.001), as well as poorer sleep quality as reflected by PSQI (p = 0.005) and ISI (p = 0.033).;Approximately one-third of patients were preventive treatment-naïve, whereas the remaining two-thirds were receiving preventive medications, most commonly flunarizine and/or topiramate. ‖ 结论:Taken together, these findings shift attention from headache-specific metrics alone toward neuropsychiatric and lifestyle factors that may be relevant to structural brain-age deviation in migraine.
局限
Furthermore, the elevation was also not specific to a particular migraine subtype, as both episodic and chronic migraine showed a higher BAG than controls, though the small chronic subgroup (n = 24) makes this comparison preliminary.;Notwithstanding this offset, the cross-cohort convergence and the robustness to neuropsychiatric adjustment suggest that the elevated BAG in migraine may reflect a reproducible structural deviation from the normative aging trajectory, although the modest and unbalanced control sample in the present cohort (see Limitations) warrants confirmation in larger, more balanced case-control designs.;Whether active management of depressive symptoms might benefit long-term
