研究问题
Similarly, clinical confounders such as objective cognitive impairment, specific medical comorbidity burdens, extrapyramidal symptom severity, nutritional status, and precise physical activity levels were not comprehensively evaluated.
方法
Furthermore, the prevalence of frailty increases significantly with age [16], accompanied by elevated risks of falls, disability, and hospitalization [17, 18].Currently, research concerning schizophrenia patients and their frailty status remains limited [19, 20].;Data from a study on frailty in patients with schizophrenia indicated that 8% were at high risk of frailty and 4% were at moderate risk; notably, those at moderate-to-high risk showed a significantly increased rate of in-hospital mortality [22].;Specifically, active psychiatric symptoms (as measured by the PANSS) and general psychological distress (measured by the BSRS-5) directly diminish a patient’s motivation and self-care abilities, while baseline physical dependency levels (measured by ADL and IADL) serve as critical indicators of functional reserve [25, 27].
核心结论
This indicates that the structural shift in the elderly population is exerting significant pressure on both the healthcare and social systems [3].Population aging is accompanied by multiple health challenges, including frailty, sarcopenia, cognitive impairment, and dementia, all of which are closely linked to physiological decline [4].;Research has demonstrated that frailty is not only significantly associated with functional decline, falls, disability, and mortality risk, but also increases hospitalization rates, healthcare resource utilization, and the burden of long-term care, imposing heavy costs on individuals, families, and society [2, 9].Research indicates that the prevalence of frailty among community-dwelling older adults ranges approximately from 12% to 30% and is significantly correlated with malnutrition, depressive symptoms, a history of falls, hypertension, and multimorbidity [10–12].;Although Fried’s phenotype comprises operational performance criteria rather than a traditional psychometric scale—making standard internal consistency metrics such as Cronbach’s α less applicable—these five components have been extensively validated in Taiwanese epidemiological studies, demonstrating robust predictive validity for adverse health outcomes [33].
局限
However, these correlations are relatively weak, with all correlation coefficients (r) involving frailty severity being less than 0.4.Regarding psychological distress (BSRS), it shows significant positive correlations with all other psychiatric-related questionnaires; specifically, it exhibits a moderate correlation (0.4 < r < 0.7) with general psychopathology symptoms (r = 0.56, p < 0.01), while only low correlations are found with the remaining scales.;However, these reported AUC estimates represent optimistic in-sample discrimination evaluated on the development dataset, and future external validation in independent cohorts is warranted (See Table 4).DiscussionThe prevalence of frailty among the participants was 55.5% (86/155).;Future studies with larger sample sizes and comprehensive glycemic indicators (such as HbA1c) are warranted to further elucidate the complex relationship between metabolic profiles and frailty in chronic schizophrenia [47–50].In the schizophrenia population, negative symptoms and general psychopathology symptoms are traditionally considered strongly correlated with functional decline and are highly correlated with frailty [16, 25, 26].
