研究问题

Hearing loss (HL) among adults and older adults is increasingly understood as a multidimensional condition that contributes to a wide spectrum of adverse outcomes, encompassing core auditory challenges like communication difficulties as well as broader psychosocial and health consequences [-].;Despite these advances, significant gaps remain in both the ICF-CSHL and the HEAR-COMMAND Tool.;This underrepresentation is evident in the Comprehensive ICF-CSHL, which contains only one category addressing balance and mobility challenges—“vestibular functions” (b235), defined as “sensory functions of the inner ear related to position, balance, and movement, including functions of position and positional sense, balance of the body, and movement” [].

方法

Participants were classified into three groups: 1) normal hearing, defined as a PTA4 of ≤20 dB HL; 2) individuals with HL, unaided, defined as a PTA4 of >20 dB HL and no current use of hearing aids; and 3) individuals with HL, aided, defined as a PTA4 of >20 dB HL and current use of hearing aids.;Within the sample, 25% (n=18) were normal hearing individuals, 40% (n=29) were hearing aid users with mild to moderately severe HL, and 35% (n=25) were nonhearing aid users with mild to moderately severe HL.;Participants were purposively sampled from the Speech and Hearing Clinic at Auburn University and had completed the HEAR-COMMAND Tool as part of its validation study [].;All 72 participants were additionally asked to complete the Balance and Mobility Scale (BMS) to assess balance-mobility difficulties.

核心结论

This indicates strong inter-item correlations and confirms that the 13 items collectively assess a unified construct of balance-mobility challenges.;The Kaiser-Meyer-Olkin measure (0.86) and Bartlett’s test of sphericity (χ2(78)=642.25, p<0.001) confirmed the suitability of the data for factor analysis with the sample size.;Item Q6 (movement coordination) demonstrated substantial cross-loading on both factors (0.76 and 0.68), suggesting it relates to both physical and perceptual aspects of mobility. ‖ 结论:These findings persist across hearing status groups, highlighting a critical gap in the current practical application of the ICF-CSHL frameworks and tools like the HEAR-COMMAND, where these domains remain underrepresented.

局限

Notably, this persistence of functional challenges was further reflected in the multivariate regression analysis, which provided deeper insight into the factors influencing self-reported hearing disability.;This finding suggests that self-reported hearing disability is influenced more robustly by balance-mobility challenges and perceptual experience than by conventional audiologic rehabilitation, which is often heavily focused on technological solutions (e.g., hearing aid use) or delivered within siloed clinical models.;First, even with optimal amplification, many individuals experience persistent residual disability in complex or noisy listening that demands significant cognitive and postur
