研究问题

15 – 17 Without separating the future burden into demographic effects and changes in intrinsic risk, health systems cannot judge whether to prioritise expanding emergency and inpatient capacity or to invest more intensively in preventive strategies.;14 The disproportionately high burden of vertebral column injuries in non-metropolitan areas suggests exposure to higher-energy falls, a hypothesis supported by differences in fall location.

方法

14 In Korea, existing studies on falls among older adults have relied predominantly on community surveys or small cohorts, leaving ED-based burden, geographic disparities and future trajectories largely unexamined.;Statistical analysis Demographic and clinical characteristics were compared between metropolitan and non-metropolitan areas and across population density tertiles using Pearson’s χ 2 tests.;Recent generational comparisons reveal that newer cohorts are physically more active but report higher frequencies of falls.

核心结论

The COVID-19 pandemic introduced a structural break that complicates simple extrapolation of pre-pandemic trends, as mobility restrictions, altered care-seeking behaviour and excess mortality among the oldest-old may have collectively reset the baseline risk trajectory.;Our data show that this subgroup has the steepest increase in visit rates, indicating a compounding effect of frailty and social isolation that increases exposure to indoor hazards.;42 43 Our findings suggest that health systems across East Asia may confront not only demographic growth in older populations but also potential non-linear increases in intrinsic risk.

局限

4 However, important uncertainties remain regarding the true trajectory of this burden.;Survey-based data are vulnerable to recall limitations and often fail to represent the clinical reality seen in emergency departments (EDs), where increases in acute and severe fall-related injuries have been reported.
