研究问题

However, whether these endothelial-protective effects delay the transition from NPDR to PDR remains unproven and requires validation in diabetes-specific retinal models.58,59Hypoxia-related signaling: In retinal pigment epithelial cell models exposed to hypoxic stimulation, formononetin has been reported to reduce HIF-1α- and VEGF-associated responses.;In RPE cells, formononetin reduced VEGFA and HIF-1α-associated responses under hypoxic stimulation, and OIR studies suggest anti-neovascular activity.17 However, whether this upstream modulation translates into clinically meaningful benefit in diabetic retinopathy remains unknown and requires validation in diabetes-specific retinal models.60Protection of the Neuro-Vascular Unit (NVU): Formononetin has been reported to exert anti-inflammatory and antioxidant effects via the JAK2/STAT3 and NF-κB axes.;However, these findings remain limited to preclinical models, and the relative contribution of microvascular protection versus direct neuronal or Schwann-cell effects remains unclear.64–66Mechanistically, formononetin has been reported to upregulate SIRT1 and NGF expression in neural tissue.

方法

Among them, oxidative stress and low-grade inflammation are often considered common pathological features across different microvascular beds, which supports their potential relevance as therapeutic targets.7Although control of blood glucose, blood pressure, and lipid levels remains central to the management of DMCs, many patients still develop progressive end-organ damage.8–10 Current pharmacological approaches also have limitations.;Therefore, the angiogenic activity of formononetin should not be treated as a simple contradiction.;However, these predicted indices should be distinguished from experimentally measured pharmacokinetic data.

核心结论

This similarity may contribute to its phytoestrogenic activity, whereas CYP450-mediated metabolism may affect its pharmacokinetic behavior and potential drug-interaction profile.31,32 From a drug-likeness perspective, in silico parameters, such as predicted oral bioavailability and drug-likeness scores, suggest that formononetin has some properties of a developable small molecule.;Overall, preclinical studies have reported antioxidant, anti-inflammatory, and anti-fibrotic activities of formononetin.18,33–35 Figure 2Chemical structure and representative botanical sources of formononetin.;Available studies in high-glucose-treated renal cells, STZ-induced diabetic rodents, and db/db mice suggest that formononetin may attenuate renal oxidative stress, mitochondrial dysfunction, inflammatory activation, and fibrotic remodeling.

局限
